Short answer: fifteen insurers sell 2026 marketplace plans in Texas, but only one — Blue Cross and Blue Shield of Texas — sells in all 254 counties. Most Texans choose between three and five insurers, seven counties have only one, and there is no PPO anywhere on the Texas marketplace. What That Means Where You Live
The statewide list overstates your choice. The big metro counties get the most competition — nine insurers in Dallas and Travis counties, eight in Bexar and Tarrant, seven in Harris — while 7 counties have a single insurer and 33 have two. In the median county the benchmark Silver plan for a 40-year-old costs about $724 a month before any tax credit, from $564 in Cottle County to $963 in Throckmorton County.
Every plan on the Texas marketplace is an HMO, EPO or POS. HMOs and EPOs pay nothing out of network outside an emergency, and a POS pays part of an out-of-network bill, so the network matters as much as the insurer. Check that your doctors and hospital are in the specific plan, not just the company.

Changes Coming for 2027
Cigna leaves the ACA individual market for 2027, Texas included, so its members in those 11 counties need a new plan for January 1 (who is leaving Texas). The full 2027 insurer list comes out when CMS posts the 2027 plan files in late October; this page will be updated then. Open Enrollment runs from November 1, and choosing by December 15 gets you coverage from January 1.
Private Health Insurance in Texas
“Private” covers two different things. Marketplace plans are private insurance too — the companies above — and the same ACA plans are also sold outside HealthCare.gov, without tax credits. Separately, some coverage is medically underwritten: the insurer asks health questions and can decline or exclude conditions. For a healthy household above the subsidy line that can price well; for anyone with a real health history, the marketplace rules protect you and are usually the right place to be. Medically underwritten coverage is not available in every state and acceptance depends on health history.
