Most insulin assistance programs are set at 400% of the federal poverty level. The threshold for each brand, income limits by household size, and what the programs require.

Nearly every insulin brand has a manufacturer assistance program, and nearly all of them draw the income line in the same place. Here is the threshold by brand, and what qualifying actually takes.

Insulin is the most consistent category on the whole assistance list. Where other drug classes scatter between 100% and 600% of poverty, the insulins cluster at 400% — under $63,840 for one person, under $132,000 for a family of four.

Income Is Not the Only Test

These programmes also require that you have no other way to pay for the drug — you are uninsured, or your plan does not cover it — and many exclude people on Medicare or Medicaid. Assistance is not insurance and does not count toward a deductible or out-of-pocket maximum. Full eligibility rules are on the overview.

Figures use the 2026 federal poverty guidelines — a different table from the one behind marketplace subsidies, so do not use one to estimate the other. More on the basis here.
